Taken out of current rumors, the iWatch could let the user identify incoming calls on the iPhone. Next up is the usual ability to monitor heart rates.
If your iPhone is in your pocket while you walk, a vibrating iWatch would ask for some attention. The iWatch would show you the face of the person calling you on your iPhone. Hitting the answer button would engage the conversation. Very simple and effective usage scenario.
